Dementia Care

In order for us to deliver the right care to someone with dementia, we require an up-to-date diagnosis and medical assessment.

Once this diagnosis has been made, we put together a person-centred care plan. When drawing up this care plan, we liaise with the patient, their family and friends to create a clear image of their life history, hobbies and aspirations.

Upon referral from a GP, Admiral Nurses are able to assist us in caring for you and accompany you to the memory clinic.

We are currently working towards building a sensory garden in an enclosed outside area, and a purpose-built secure dementia care unit inside the Home.
